EXPECTATION OF YOUR COMMITMENT
I understand it is expected of me to fully cooperate with the ministry team and with the Holy Spirit in order to receive God’s help. The team may ask me to pray, fast or do some outside “homework” in conjunction with my ministry time.
They also may ask me to be accountable to them in some specific area of my life because of certain behaviors.
WAIVER OF LIABILITY & CONFIDENTIALITY
I understand that I will be meeting with Ministers from Lionheart Church who will help me overcome my problems and grow in my Christian walk. I accept that some may not be licensed counselors and that they may or may not be ordained Ministers. I accept that all counseling is based on the Bible and all personal Ministry is under the direction and control of the Holy Spirit.
I accept that no person working with or at Lionheart Church can guarantee that I will receive any particular healing or result. This I waive all rights to claim of liability.
I accept that all statements whether oral or written will be held in confidence by the Minister and that legally information will not be disclosed without my oral or written consent. I do however waive my right to complete confidentiality in the following situations.
1. The Minister may give a verbal or written summary’s report to the Director of the Inner Healing Ministry.
2. The Minister or any other person involved in working with adults and children are either encouraged or required by law to disclose to certain agencies or authorities any potential harm that a person may do to themselves or to others.
3. The Ministers of Lionheart church are required by law to report any reasonable suspicion of physical or sexual abuse of children.
Sessions, whether conducted in person or via Zoom, are recorded for training purposes and may be shared with members of the Lionheart Inner Healing & Deliverance Team.
