  • Welcome to the Hawaiʻi OST Quality Self-Assessment!
    This tool is designed to help you reflect on your program's strengths, identify growth opportunities, and create a meaningful action plan for improvement.

    What to Expect:

    • Quality Guidelines: You will rate each indicator on a scale of 1 to 5, followed by a brief reflection for each guideline.
    • Automatic Scoring: You will see your calculated totals and averages automatically generated in a visual summary table.
    • Final Reflection: You will end with overall reflections to capture your main takeaways.
    • Your Customized PDF: Once finished, you will be able to immediately download a personalized, fully mapped PDF report of your results and reflections.

  • Programs do not need to address every indicator at once. Teams are encouraged to focus on one or two priority areas. This self-assessment is an opportunity to reflect honestly on your program's practices, recognize what's working well, and identify opportunities for growth. There are no right or wrong answers, only a chance to learn and improve. Your input will also help us refine this tool before it is shared more broadly across the state.

    2. Planning: The program is discussing or preparing to implement the practice.
    3. In Progress: Initial steps are underway, but practice is not yet consistent.
    4. Implemented: The practice is consistently applied across the program.
