Summer Camp Liability Waiver
Jannah Circle Inc NFP (henceforth referred as JC) and our Summer Camp (henceforth referred as SC) are committed to conducting its recreation programs and activities in a safe manner an holds the safety of participants in high regard. JC and SC continually strive to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants' safety. However, participants and parents/guardians of minors registering for programs /activities. You are solely responsible for determining if you or your minor child/ward are physically fit and/or skilled for the SC activities. It is always advisable, especially if the participant in any way has suffered an illness, injury, or impairment to consult a physician before undertaking any physical activity. You're solely responsible for informing the JC and SC staff if your child has any allergies and provide instructions for the staff in case of emergency.
You will not attempt to record, electronically or otherwise, the audio or video of any class or sessions in part or full. You agree that all content from SC workshop is the intellectual property of JC and SC, and it cannot be retaught, re-purposed, or utilized in personal, local, or online teaching platforms without the written consent of the Director of the program. You understand that failure to comply may result in the removal from the program.
Recreational activities are intended to challenge and engage the physical, mental, and emotional resources of each participant. Despite careful and proper preparation, instruction, medical advice, conditioning, and equipment, there is still a risk of serious injury when participating in any recreational activity.
Understandably, not all hazards and dangers can be foreseen. Depending on the particular activity, parents must understand that certain risks, dangers, and injuries due to inclement weather, slip and falls, poor skill level or conditioning, carelessness, horseplay, unsportmanshiplike conduct, premises defects, inadequate or defective equipment, inadequate supervision, instruction or officiating, and all other circumstances inherent to indoor and outdoor recreational activities exist. In this regard, it must be recognized that it is impossible for JC, SC, and its staff and volunteers to guarantee absolutely safety.
Summer Camp is NOT a day-care center. They cannot provide child care services. If, in the parent's determination, a child is not old enough to go to and from Jannah Circle on his/her own the parent/guardian is responsible for assuring the child's arrival and departure. Jannah Circle Inc NFP, The Jannah Circle Community Center, and Summer Camp is not responsible for ensuring that any child stays on the premises. The staff or instructors are not liable for any Summer Camp attendee outside the Center facility before the student enters or after the student leave the Center.
Please read this form carefully and be aware that in signing up and participating in Summer Camp, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages, or loss which you or your minor child/ward may sustain as a result of participating in any and all activities connected with and associated with these programs. You recognize and acknowledge that there are certain risks of physical injury to participants in the Summer Camp program, and you voluntarily agree to assume the full risk of any and all injuries, damages, or loss, regardless of severity, that your minor child/ward or you may sustain as a result of SC participation. The parent/guardian shall be liable for any damages and injuries caused by their child to other persons and the parent agrees to hold harmless, defend, and indemnify Jannah Circle Inc NFP and Summer Camp and its staff and instructors. You further agree to waive and relinquish all claims you or your minor child/ward may have (or accrue to you or your minor child/ward) as a result of participating in these programs against JC and SC including its officials, agents, volunteers, and employees.
