Southern Pets Animal Rescue (aka SPAR) is a rescue organization for homeless animals. It is our goal to place these animals in permanent, loving homes where every member of the household is committed to their lifelong care. If you are willing to take on such a responsibility, please complete the application below.
Adoption Requirements
You must be 21 or have the consent of your legal guardian. Proof will be required.You must be willing to allow a representative of SPAR to make a virtual home visit, follow-up visit in person, or by phone. Random visits may be made anytime within the first 60 days of adoption.You must pay the standard adoption fee at time of adoption approval and retrieval of the animal.You must provide the name and phone number of your veterinarian and make a visit within the first 30 days of adoption. If you currently do not have a veterinarian, you will need to establish one within this timeframe.

Agreement to Adopt
By his/her signature below, adopter hereby agrees to the following terms and conditions. Please read thoroughly.Adopter acknowledges that SPAR is providing an animal for adoption on an "as is" basis. SPAR makes no representation as to the temperament or medical condition of the animal being adopted. All medical records received by SPAR are provided to adopter.Adopter shall have the animal established with personal veterinarian within 30 days of adoption, which will include exam and acknowledgment of previous vet history provided by SPAR to adopter.Adopter shall allow a representative of SPAR to visit the animal on a random basis during the first year of adoption and to notify SPAR of any change of address during this time period. Should SPAR find that the animal is not being cared for in compliance with this agreement, or in violation of local or state laws, SPAR reserves the right to seize the animal for the safety and welfare of the animal. This includes compliance with any voucher provided for dated vet services.Adopter shall provide for animal’s physical and emotional needs including proper diet and nutrition, housing, exercise, medical care, vaccinations, and monthly heartworm preventative.Adopter shall comply with all state and local laws pertaining to the care and keeping of the animal, including leash laws, licensing, registration, and rabies vaccinations.Adopter agrees not to abandon, give away, sell or trade the animal for the duration of the animal's natural life.Adopter agrees to contact SPAR in the event that the animal must be rehomed due to unforeseeable circumstances. Under no circumstances will the animal be returned to PetSmart or any other adoption event facility, nor surrendered to a shelter or other rescue group. SPAR reserves the right to accept or decline the return of the animal at its own discretion.Adoption fee will not be refunded after 72 hrs. unless adopter contacts rescue within that period requesting additional time to work through any issues. Length of extension will be on a case by case scenario and communicated directly with the adopter and President of SPAR.Adopter agrees to make every reasonable effort to locate the animal should it escape or become lost or stolen, including contacting SPAR as soon as possible. SPAR will provide information and/or assistance to help locate the missing animal.On the effective date of this adoption agreement, adopter assumes complete liability for the adopted animal.In the event that SPAR deems it necessary to contact an attorney to enforce the terms of this agreement, adopter shall be responsible for the payment of attorney's fees, court costs, and/or fines necessary to enforce this agreement.It is the requirement of this rescue that ALL animals adopted be spayed or neutered by 6 months of age. If you have adopted a puppy too young for this procedure, you will be given a voucher to be redeemed at a participating SPAR veterinary facility. If the adult dog you have adopted has a scheduled spay or neuter appointment, you are required to honor this appointment. If an appointment has not been scheduled, or procedure not performed due to the health of the animal, you will have 45 days from date of adoption to comply. Should the procedure not take place within this time frame, you are in violation of this contract and SPAR will take the necessary steps to reclaim the animal. Adoption fee will not be refunded. Puppies/kittens require a series of 3 vaccines spaced 3 weeks apart. This is included in your adoption fee. The 1st set of vaccines is given at 6 weeks of age, along with the initial fecal test and deworming. All subsequent fecal tests and deworming medication, if needed, is the responsibility of the adopter. The 4th set of vaccines is a booster shot and is at the adopter’s expense, if suggested by the veterinarian. Should the adopter choose to have the remaining shots at another veterinary clinic other than the one approved by the rescue, payment responsibility is that of the adopter. Adopter must submit proof to SPAR that this service was performed within the proper timeline. If SPAR is unable to perform a reference check prior to preliminary approval of this application and the animal leaves the SPAR foster, SPAR reserves the right to reclaim the animal pending any detrimental or false information on contract, or received regarding the welfare of this animal, or if we feel the placement is incorrect. In the event SPAR is informed, or becomes aware of mistreatment to the adopted animal, lack of routine vet care or heartworm prevention protocol, proper grooming, adverse living conditions, or any other condition detrimental to the animal, we reserve the right to rescind the adoption if proper measures are not taken to immediately rectify the situation. All costs related to the retrieval of the animal in question will be at the adopter’s expense. *Initial below - REQUIRED

I, the undersigned adopter, have read and acknowledge the terms of this agreement. I understand I will receive a copy of all medical records available for the animal I have chosen to adopt. Effective upon date of contract, I, adopter, assume complete liability for the animal and agree to hold SPAR harmless for any financial, medical or legal liability for the animal. I agree to abide by all conditions listed within this Agreement and to maintain contact with SPAR during the first year of adoption to help ensure a smooth transition of this animal to my family.I UNDERSTAND THIS APPLICATION IS NOT FORMERLY APPROVED AND THIS CONTRACT DOES NOT BECOME FINAL UNTIL SPAR HAS PERFORMED THE NECESSARY REFERENCE CHECKS TO VERIFY THAT THE INFORMATION PROVIDED HEREIN IS ACCURATE. IF YOUR CURRENT PET, OR PETS, OWNED WITHIN THE LAST 5 YEARS, HAVE NOT BEEN, OR ARE NOT CURRENT ON VACCINATIONS AND MONTHLY HEARTWORM PREVENTION, YOUR APPLICATION WILL BE DENIED AND ADOPTION VOIDED. IF THE ANIMAL IS ADOPTED ON A WEEKEND OR AFTER HOURS, SPAR WILL CONTACT YOUR VETERINARIAN WITHIN THE TWO FOLLOWING BUSINESS DAYS TO CONFIRM. I UNDERSTAND I AM RESPONSIBLE FOR RETURNING THE ANIMAL TO THE RESCUE WITHIN 48 HOURS OF INITIAL NOTIFICATION SHOULD MY APPLICATION BE DENIED FOR THE ABOVE REASONS. IF THE ANIMAL IS NOT RETURNED, I UNDERSTAND STEPS WILL BE TAKEN TO RECLAIM THE ANIMAL ON BEHALF OF THE RESCUE.
