Camp Transportation Waiver

Please fill out this form to authorize travel for your child.
TRANSPORTATION LIABILITY WAIVER - My son/daughter has my permission to ride to and from SonRise Bible Church in Atkinson, IL and Camp Manitoqua in Frankfort, IL from January 2nd - 3rd, 2026. I do release SonRise Bible Church, their ministers and volunteer leadership, from liability for injury or accident, and do give my permission to the church’s leadership to administer basic first aid and secure proper medical attention should the need arise for the duration of the event.

PHOTO RELEASE WAIVER - I also grant SonRise Bible Church permission to usephotographs and/or video taken of my son or daughter at, in the vehicle to and from Camp Manitoqua, and while at the camp either of the above mentioned dates.
