Dog Boarding Form

FosterDoc Houston Dog Boarding Agreement & Liability Waiver This Boarding Agreement & Liability Waiver is entered into by and between FosterDoc Houston ("Facility") and the pet owner ("Owner") regarding the boarding of the Owner's dog(s). By submitting this form and signing below, Owner agrees to all of the following terms and conditions: 1. OWNERSHIP & AUTHORITY: Owner represents that they are the legal owner or authorized guardian of the dog(s) listed in this form and have full authority to enter into this Agreement. Owner confirms that the dog(s) are in good health and have not exhibited aggressive or threatening behavior towards any person or other animals. Owner certifies that all information provided in this form is accurate and complete. 2. EMERGENCY MEDICAL CARE & OWNER RESPONSIBILITY: If the dog(s) becomes ill or requires medical attention while in the care of the Facility, staff will make every reasonable attempt to contact the Owner and designated emergency contact. If the Owner is unreachable, Owner hereby authorizes Facility to proceed with life-saving measures and emergency veterinary care at the Facility's sole discretion. Owner agrees to be solely responsible for any and all costs incurred for veterinary, medical, or other treatment rendered during the boarding stay. Owner agrees to hold harmless and release Facility, its employees, volunteers, and agents from any and all liability, claims, suits, or damages arising from illness, injury, or death of the dog(s) while in Facility's care. 3. ABANDONMENT POLICY & FINE: Owner understands and agrees that they retain full legal ownership and financial responsibility for the dog(s) at all times during and after the boarding stay. FosterDoc Houston is a rescue organization and does not accept surrendered dogs through the boarding program. If Owner fails to pick up the dog(s) within 7 days after the scheduled check-out date without contacting the Facility, the dog(s) will be deemed legally abandoned. In the event of abandonment, Facility reserves the right to place the dog(s) with a rescue group, shelter, or other appropriate placement. Owner agrees to pay an abandonment fine of $1,000.00, in addition to all accumulated boarding fees and any veterinary or care costs incurred. Facility reserves the right to pursue legal action to recover all fees and costs. 4. LIABILITY FOR BEHAVIOR & DAMAGES: Owner understands that dogs in a boarding environment may interact with other dogs, and that minor cuts or scratches can occur even with careful supervision. Owner agrees to be solely responsible for any harm, injury, or property damage caused by their dog(s) to other pets, Facility staff, volunteers, or Facility property. Facility reserves the right to permanently remove any dog from boarding if it displays aggressive or dangerous behavior. 5. VACCINATIONS & HEALTH: Owner certifies that the dog(s) are current on all required vaccinations and that proof has been provided. If the dog(s) is found to have fleas, ticks, or internal parasites upon arrival or during the stay, Facility may administer appropriate treatment at the Owner's expense. By signing below, I confirm that I have read, fully understand, and agree to all terms, conditions, and statements of this Agreement.
