Virtual Course Waiver, Release of Liability & Media Consent

1. Virtual Participation Acknowledgment:

I understand that I am enrolling in virtual online course provided by The MONARCH ELEVATION Project which may include movement-based, weight training, mindset, and empowerment exercises conducted over virtual platforms such as Zoom, FaceTime, or Google Meet.

I understand that participation in these classes involves physical and mental activity and carries with it the risk of injury, whether physical or emotional. I affirm that I (or my child) am in proper physical and mental condition to participate in these sessions.

2. Waiver and Release of Liability:

By signing below, I voluntarily release, waive, discharge, and hold harmless Motivate & Elevate Coaching By Rebecca & Amber Conner, Monarch Fitness & any coaches, agents, and any associated individuals or organizations from any and all claims, liabilities, or demands arising out of or in connection with participation in the virtual coaching sessions, including any injury, loss, or damage that may be sustained while participating.

I acknowledge that it is my responsibility to provide a safe and distraction-free environment for myself &/or my child participation in virtual classes.

3. Media Release:

I hereby grant permission MOTIVATE & ELEVATE Coaching By Rebecca, MONARCH FITNESS, & The MONARCH ELEVATION Project to record, photograph, or screenshot portions of virtual sessions that may include myself or my child. These materials may be used for:
    •    Promotional materials
    •    Social media posts
    •    Training and educational resources
    •    Program documentation

I understand that these images or videos may be used without further notification, and that no personal information will be shared without consent beyond first name or general group context.

4. Consent and Signature:

I certify that I have read this document and fully understand its contents. I am signing this agreement voluntarily and acknowledge that this release of liability is binding.
