- An invoice will be issued once this first intake form has been received and approved.
- A 50% deposit is required upon approval.
- Invoices will be sent to the email address provided in this form.
General Information
- To select a participation option, tick the small white box in the top left corner of your choice. Your selection will automatically appear in the shopping cart at the bottom of this form.
- Once your registration is approved and the deposit received, a second intake form (contract) will be sent to collect additional details, such as SKU information, Masterclass thematic, Power Lunch attendee names etc.
