Prep Team Uniform Order Form – Important Info

✅ PREP TEAMS:

If your athlete is not on one of the teams listed above, please do not complete this form.

➡️ All PREP athletes must rent a uniform. 

➡️PREP athletes must buy a comp bow (females only). 

➡️PREP athletes may wear any all white indoor cheer shoes. The shoes on this form are one option. 

➡️ Prep Athletes must purchase a lip stain. This is the same color as last year. It only needs to be purchased if your athlete needs a new one. 

📏 Uniform Sizing
Sizing will be done in person at the gym.
There is no place to enter sizes on this form.

💳 Payment
Your card will be charged upon submission of this form.

👕 Practice Wear (Required)
Prep Athletes receive a sponsor shirt, included in their tuition. All other practice wear is optional.

Please DO NOT order parent / spectator shirts on this form. A separate form will be sent. 

Uniform Rental Agreement: 

 The uniform consists of the Top and a skirt. (Top and Pants for Male Athletes)
The uniform is the property of Rock Solid All Stars.
By signing this agreement, you understand the following responsibility of renting this uniform.
1. READ THE TAG – AND FOLLOW DIRECTIONS THOROUGHLY PLUS A FEW ADDITIONAL
STEPS LISTED BELOW.
2. WASH UNIFORM SEPARATELY. THAT MEANS TOP AND SKIRT BY ITSELF NO OTHER
CLOSTHES IN THE WASHER! DO NOT DRY CLEAN – DELICATE CYCLE IS BEST.
3. USE A FULL LOAD OF WATER – SO THE WHITE DOES NOT GET DINGY GRAY!!
4. USE A SMALL DRIP OF SOAP – TOO MUCH SOAP YELLOWS THE WHITE FABRIC OVER TIME.
5. HANG TO DRY
6. WASH ASAP AFTER A COMPETITION. DON’T LET IT SIT UNTIL THE NIGHT BEFORE THE
NEXT COMPETITION. YOU WOULD NOT BELIEVE THE NUMBER OF UNIFORMS TURNED IN
LAST YEAR THAT WERE NEVER WASHED!!!!
7. DO NOT GET A SPRAY TAN BEFORE ANY COMPETITIONS OR WHEN YOU ARE REQUIRED
TO WEAR THE UNIFORM.
8. DO NOT WRITE ON THE TAG—FIND ANOTHER WAY TO LABEL. USING MASKING TAPE IS
OK TO WRITE NAME ON LABEL – JUST WRAP A PIECE OF TAPE AROUND THE TAG AND PUT
YOUR NAME ON IT.
9. WE MUST BE ABLE TO KNOW THE SIZE OF EACH PIECE – SO PLEASE DO NOT REMOVE THE
TAG!! IF IT ITCHES OR STICKS OUT – TAPE IT DOWN.
10. DO NOT ALTER THIS UNIFORM—IF YOU DAMAGE, ALTER OR LOSE THE UNIFORM YOU
ARE REPSONSIBLE TO PAY FOR IT. IF THERE ARE ANY PROBLEMS WITH YOUR FIT – BRING
IT IN AND WE WILL DO OUR BEST TO TAKE CARE OF THE PROBLEM.
11. WE AGREE TO RETURN THE UNIFORM IN GOOD CONDITION TO ROCK SOLID ALLSTARS AT
THE END OF THE SEASON OR UPON TERMINATION FROM THE TEAM FOR ANY REASON.
12. FAILURE TO COMPLY WITH ALL OF THE ABOVE WILL RESULT IN REIMBURSEMENT TO
ROCK SOLID FOR THE FULL REPLACEMENT VALUE OF APPROXIMATELY $475.00
