Once this form is submitted:
You’ll be contacted via email once I’ve reviewed your idea. I’ll offer a few date/time options, along with a provided price estimate. A deposit ($50 for most appointments, $100 for full-day sessions) will be required to confirm the booking. Deposits are sent via e-transfer to dxctattoo@gmail.com . Deposits are non-refundable but transferable with 48h notice.
I understand that this booking request form does not guarantee me an appointment. I understand that I will be contacted for follow up via email, and will be required to send a deposit via etransfer within 24 hours of agreeing to an appointment time with the artist. I acknowledge that until the deposit has been received my appointment has not been confirmed. I understand that this is a non refundable deposit. If I need to cancel or reschedule the appointment I will do this no later than 48 hours before my appointment to avoid losing my deposit. I understand that if I make changes to my design after submitting this request and booking the appointment the cost may vary from the quote I was given, or I may need to reschedule.
