1. Voluntary Participation
I understand my participation in Clubbin Cube golf simulator activities is entirely voluntary.
2. Assumption of Risk
I understand golf simulator activities involve certain risks, including:
- Injury from swinging golf clubs
- Contact with other participants
- Errant or misdirected golf shots
- Slips, trips, or falls
- Equipment malfunction or failure
I accept all such risks and take full responsibility for my participation.
3. Release of Liability
I release and hold harmless:
- Clubbin Cube LLC
- Its owners, employees, contractors, and partners
- The event venue and its affiliates
from any claims, demands, or causes of action for injury, property damage, or loss.
4. Participant Conduct
I will:
- Follow all safety instructions from Clubbin Cube staff
- Act responsibly
- Use equipment properly and only as intended
- Not participate under the influence of alcohol or drugs
5. Photo & Video Consent
I grant Clubbin Cube permission to capture, use, and publish my image, voice, and likeness in photos, videos, or other media for any lawful promotional, advertising, or marketing purpose, without payment or further approval. (Optional opt-out available.)
6. Medical Acknowledgement
I am physically able to participate and have no medical condition preventing safe play.
7. Legal Age & Authority
I am 18 or older and legally able to sign. If signing for a minor, I am their parent or guardian.
