    NOTE - failure to accurately provide the information below on this form may delay your application review, and could result in denial for participation in the program.  

    To successfully complete this form, please ensure you have the following:

    • The student and parent/guardian must complete the form together.
    • Both must sign the Aiken iNNOVATE contract agreement.
    • Parent's email address
    • Student's email address
    • School counselor's email address
    • Student’s PowerSchool ID or Lunch Number
    • If applying from a non-ACPSD school, you must upload the student’s most recent grade report (accepted formats: scanned PDF or screenshot).

  • Start Dates and School Information

  • Aiken iNNOVATE allows for rolling enrollment throughout each semester.  Start dates are on Mondays, and students are typically able to start within two-three weeks after applying, if they are found eligible for participation.  

    If the student has accommodations through an IEP or Section 504, or an MLP, a special review is required prior to starting the onboarding process.  

    Note: Applications submitted in the last 4 weeks of the semester will have a start date of the following sememster.

  • It is our goal to start a student as soon as possible unless you indicate something different below.  Usually, this is the second Monday after applying and being invited to participate.

  • For the next question, you must indicate the student's school counselor email.  If the student is not an Aiken County Public School student, please put N/A for this.  

    For questions and clarification about your students assigned counselor, please contact your home school. 

    We have provided a list of school counselors for your convenience.  

  • Most Recent Report Card or Grade Report

  • Your application cannot be submitted without eligibility documentation.  If you need more time, you can save your submission and return at a later date to complete the form.  

    Please submit a current grade report for your student.  The report must include both grades and attendance data. 

    This can be the student's most recent report card, a screenshot of a current grade report that shows semester grades, or some other kind of document that can give us data to assess student eligibility for our program.

  • There is no need to re-apply.  We are planning to have you back with us unless you email us requesting to return to in-person learning.  Please close the survey and do not submit.
