NOTE - failure to accurately provide the information below on this form may delay your application review, and could result in denial for participation in the program.
To successfully complete this form, please ensure you have the following:
- The student and parent/guardian must complete the form together.
- Both must sign the Aiken iNNOVATE contract agreement.
- Parent's email address
- Student's email address
- School counselor's email address
- Student’s PowerSchool ID or Lunch Number
- If applying from a non-ACPSD school, you must upload the student’s most recent grade report (accepted formats: scanned PDF or screenshot).
