I am aware and understand that there are a number of inherent risks involved in my child's and mine participation in dancing and twirling activities. These risks are beyond the control of the Canadian Baton Twirling Federation, the Provincial Baton Twirling Organization, Association of Corporation in the province, as well as Fraser Valley Baton and Dance, in which any competition and/or sanction activity in which myself or my child participate takes place, the host club(s) or province, the event or competition director, the owner, occupier, operator, and/or tenant of the event facility, and any person, corporation or organization associated with the operation of such competitions or events. I further agree that I am assuming personal responsibility for any costs as well as any loss, damage, injury, or ambulance service resulting from or in connection with such participation at each competition or event in which me or my child takes part, or an event in which me or my child participate. I have read and understood this waiver.

During the year, photographs of dancers and twirlers may be taken during competitions, banquets, clinics, awards and special presentations or other events. Your consent is required or a parental/guardian consent if athlete is under the age of majority. Media coverage is occasionally done throughout the season to promote the sport of Baton Twirling, the studio and dance, as well as to acknowledge the club and athletes/dancers accomplishments. This means publishing of athlete’s/dancers names, and club associations in the local newspapers, the CBTF website and club website and social media pages. On occasion the media will attend competitions and events including radio, television, newspaper etc. At these times, it will be the parents’ responsibility to provide parental consent to the specific media outlets. By signing this form you are giving your consent to Fraser Valley Baton and Dance to have yours and your childs photos, names and club association be published in all cases listed above.
