I/(we) hereby authorize White Mountain Lumber LLC to store my/(our) credit/debit card ending in Last 4 Digits of Card Number* . I further authorize White Mountain Lumber LLC to make repeated and/or unscheduled charges to my/(our) credit/debit card for further purchases that I verbally or otherwise authorize from time to time and, if necessary, initiate adjustments for any transaction errors. This authorization will remain in effect until White Mountain Lumber LLC is notified by me to cancel this authorization. I further understand and agree to abide by White Mountain Lumber LLC refund policy related to any purchases made with my credit/debit card on file. Any future changes made to this agreement will be sent via email to the address listed above.

It is hereby certified that the statements in this application are true and complete. By the signature below, the Purchaser hereby agrees to pay all invoices when same become due and payable pursuant to the terms of sale. White Mountain Lumber LLC standard terms are per invoice. The Purchaser further agrees to pay a finance charge of 1.5% per month, which is a per annum rate of 18%, on past due balances of thirty days or more. In the event of default, the Purchaser also agrees to pay all costs of collection, including attorney’s fees. The undersigned waives notice of acceptance, notice of non-payment, protest, and notice of protest with respect to the obligations covered herein.

I hereby authorize White Mountain Lumber LLC to make such credit investigation, including contacting the above trade references and banks and obtaining credit reports. I further authorize all trade references, banks, and credit reporting agencies to disclose any and all information concerning the financial and credit history of the applicant as indicated above.
