The information you share below is being provided to determine eligibility to participate in the Habitat program and is NOT the actual program application.

Once you submit your Eligibility Form, you will receive an email either inviting you to apply for the program or providing reasons why you do not qualify. Please allow a few days to a week to hear from us.
