Please fill out this form anytime you have an absence outside of the school day, even if has been excused through the school. Please be sure to fill out the correct date for the absence. This will help us maintain accurate records and in turn, ensure that we enter your grade correctly.

Please note that filling out this form does not automatically excuse an absence. Just as when you miss school, you must provide specific details for us to determine whether or not an absence is excused. Absences will be excused in accordance with HCISD policy and the Hays High School Band handbook.

By providing my digital signature below, I understand that this absences request may be either approved or denied by the Hays Band staff per the Hays Bands Handbook. Both the student and the parent / guardian will be notified if their request is either approved or denied. I further understand that consistent absences by my student may result in a reduction of their grade, limited participation in the show and potential removal of the program.

In addition, I understand that band and color guard operate as team activities that rely on members consistently being there for each other. It is understood that my students absence can have a strong impact on the overall success of the program and that absences should be reserved for emergency situations ONLY.
