THIS IS NOT OUR OWNERSHIP SURRENDER FORM, but this can put you in line for us to help your horses, and it helps us track your request

AS OF 4/21, Unless a situation is a dire emergency for a horse where an owner cannot afford basic care and the horse is in critical condition, A COST OF IMMEDIATE CARE DONATION will usually need to come with the horse. Please explain your situaiton in detail, so that if you cannot send your equine with a donation for their initial vet exam, we can see whether we can still help. We wish the economic giving situation was different, but at this time, it is not. We cannot always say "YES" To all requests. Submitting this request for Heart of Phoenix to accept your horse(s) or equine(s) into rescue does not guarantee we will be able to accept them. We WILL try to help you and your equines when we can, as quickly as we can.

Are you able to make a donation of at least $500 PLUS Transport or transport costs towards the equine's care? A donation helps us accept your equine(s) sooner, as this assure our required vet/farrier care is covered for the horse from the start. If you cannot donate, please explain the situation below, and we will do our best to see if we can help:
