Iron Seed Academy Sports Camp Liability Waiver & Release Form
“Strengthening Bodies and Lifting Spirits”
I, the undersigned, understand that participation in Iron Seed Academy’s Sports Camp involves physical activities including but not limited to running, jumping, strength training, agility drills, and competitive games. I acknowledge that these activities involve inherent risks of injury, including but not limited to sprains, strains, fractures, concussions, dehydration, or more serious medical conditions.
I hereby affirm that my child (or myself, if over 18) is in good physical condition and able to participate in the camp activities. I voluntarily assume all risks associated with my child’s participation in this program, whether caused by negligence or otherwise.
In the event of a medical emergency, I authorize the staff of Iron Seed Academy to act in the best interest of my child and seek appropriate medical care, including first aid, EMS transport, and/or treatment by a licensed healthcare provider. I agree to be financially responsible for any expenses incurred.
I understand that participants are expected to follow all camp rules, demonstrate respect to staff and peers, and maintain appropriate behavior at all times. Failure to comply may result in dismissal from the program without refund.
I hereby release, waive, discharge, and hold harmless Ai Akinaa LLC, Iron Seed Academy, its directors, coaches, agents, volunteers, and affiliated entities from any and all claims, liabilities, or demands arising out of or related to any loss, injury, damage, or illness that may occur during or as a result of my child’s participation in the sports camp.
