  • CEQAC may request the following supporting documents during the initial application, renewal, annual review, or audit process. Providers are encouraged to submit available items that demonstrate good-faith compliance.

    Note: Submission of all documents is not mandatory but strengthens the provider’s compliance profile. CEQAC may follow up with requests based on provider scale, modality, or identified risks.

    CEQAC does not require providers to submit formal employment or human resources policies unless applicable to their size and structure. However, all providers—regardless of scale—must be able to demonstrate that they:

    --Comply with basic labour standards.
    --Treat staff and contractors ethically.
    --Are prepared to make reasonable accommodations for learners or staff as required by Law.

     

    This statement affirms the provider’s commitment to ethical conduct, equitable treatment of staff and learners, and compliance with relevant legal obligations, scaled to the size and nature of the organization.

    Program Title – the official name of the program as it is marketed to learners.


    Credential Issued –  type of recognition granted upon successful completion (e.g., Certificate of Completion, Micro-Credential, Diploma, Degree, Badge).


    Program Delivery – program’s instructional mode (In person, Online Real-Time (Synchronous), Online On-Demand (Asynchronous), Hybrid)


    Website URL – the direct webpage where learners can find more information about the program.


    Program Length (hours) – the total instructional hours. 


    Program Cost – the full cost of the program, including all fees, tuition, and supplies.


    Eligibility Criteria / Who the Program is for – the intended learners or target audience (e.g., working professionals, entry-level learners, specific industries). Clearly define the population the program is designed to serve, with attention to prior knowledge, experience, or career stage. Include any prerequisites or entry requirements.


    Program Objective – Summarize the overall aim of the program (e.g., to build foundational skills in project management, to prepare learners for a specific certification).
