Injury and Pregnancy Waiver
I am participating at my own risk and waive all claims of every nature against the Minnesota Vixen ™, the Women's Football Alliance, Coaches, Staff, Medical Personnel, and/or anyone associated with the Team and/or League including organizers, officials, sponsors, facility and any other participating agencies with respect to any personal loss, illness, bodily injury or death resulting from participating in these activities.

At the time of registration, I will inform the organizers regarding any relevant medical condition.

I declare that I am not pregnant. If I am pregnant or become pregnant and do not know it, I agree to hold harmless the Minnesota Vixen ™, the Women's Football Alliance, Coaches, Staff, Medical Personnel, and/or anyone associated with the Team and/or League including organizers, officials, sponsors, facility and any other participating agencies for any damage or injury that may result to me or my unborn child as a result of my participating in any way with the Team.

Photo and Media release Waiver
I grant to Minnesota Vixen, the Women's Football Alliance and its sponsors and licensees the exclusive right to the free use of my name, voice, picture, video and/or likeness in any broadcast, telecast, advertising, promotion or other account of this event.

No Refunds
I acknowledge that my registration fee is non-refundable if I’m unable to attend. IF there is antoher tryout date available, the individual may contact the Vixen to change date of tryout.

All confirmed orders are final once payment is submitted. Minnesota Vixen and/or PayPal does not issue refunds.

Summary
I, the undersigned, have read the above waiver and release, and understand that I have given up substantial rights by signing it, and sign it voluntarily.

Having read this waiver and release from liability form, and knowing these facts, and in consideration of your accepting my registration and/or my child's registration, I for myself and/or my child, and anyone entitled to act on our behalf, waive and release Minnesota Vixen, volunteers, all event sponsors, their representatives and successors from all claims or liabilities of any kind arising from my participation and/or my child's participation in this event.
