By signing this form, I acknowledge and agree to the following:

Assumption of Risk:
I understand that running and physical activity carry risks, including but not limited to: falls, collisions, traffic, heat exhaustion, cardiac events, or injury. I participate voluntarily and accept full responsibility for my safety.
Health Acknowledgment: I affirm that I am physically capable of participating in run crew activities. If I have any conditions that may affect my ability to participate safely, I have consulted a medical professional.
Liability Release: I release and discharge Livity Run, Elevtion Fitness, its organizers, volunteers, and affiliates from any liability, claims, or demands arising from my participation, whether caused by negligence or otherwise.
Medical Consent: In case of emergency, I authorize Livity Run to obtain medical assistance on my behalf and accept full responsibility for any resulting costs.

Media & Intellectual Property Release

Photo & Video Use:
I understand that Livity Run may take photos or videos at group events. By participating, I grant permission for my image to be captured and used for promotional and community-building purposes, including but not limited to social media, newsletters, websites, and branded campaigns.
Ownership & Usage: All media content captured by or on behalf of Livity Run is the sole property of the organization.
1. I may not use or sell these photos/videos for personal or commercial purposes without written permission.
2. I waive any right to compensation or royalties arising from the use of my likeness.

Intellectual Property Waiver:
I waive all rights, title, and interest to any photos or videos taken of me by Livity Run. These images are the intellectual property of the organization. Unauthorized use of media owned by Livity Run is not permitted.

Opt-Out Option:
If I do not want to appear in media, I will notify the organizers in writing or Instagram direct message before attending an event. A separate opt-out form will be provided.
