I First Name Last Name understand that this form is a request to showcase your brand / designs in the JUNE 13 2026 Put on for My City Youth Fashion/Talent Show PART 7. I understand that if I am selected as a featured designer I will be provided a Look book to choose models from to showcase designs on the red carpet. I understand I am allowed to have my own models however they must register as a model through the website and follow the show process for all models. I understand that the designer fee for this production is $150 . I understand that my designer spot is not secure with out my designer deposit or full payment. After payment is made a full detailed package description email will be sent along with itinerary and designer check in dates leading up to June 13 2026. I understand that I may receive a cash prize and award for participating as a designer however I have not been promised anything by Tailor-Made by Tae LLC. I understand that Tailor-Made by Tae will promote Me as a designer on all social media platforms.
