WAIVER AND RELEASE OF LIABILITY In consideration of being allowed to participate in any way in JBA Basketball programs, including AAU season, practices, training, events, and tryouts (referred to as “Activities”), I, the undersigned, acknowledge and agree to the following: ASSUMPTION OF RISK I understand that participation in basketball involves physical activity, potential contact with other participants, and the risk of injury. I voluntarily assume all risks, known or unknown, associated with participation in these Activities. RELEASE OF LIABILITY I release, discharge, and agree to hold harmless JBA Basketball, its coaches, directors, affiliates, volunteers, and all associated personnel, from any and all liability, claims, or causes of action arising from any injury, illness (including COVID-19), accident, or loss that may occur during participation. MEDICAL TREATMENT I authorize JBA staff or emergency medical personnel to provide treatment in case of injury or emergency. I understand that I am responsible for any and all medical expenses incurred. INSURANCE I understand that JBA Basketball carries general liability insurance for its operations and facilities. However, JBA does not provide individual medical or accident insurance for participants. It is my responsibility to ensure my child has appropriate health insurance coverage in case of injury. MEDIA RELEASE I give JBA Basketball permission to use any photos or videos of the participant taken during Activities for promotional or social media purposes, without compensation. I acknowledge that I have read and understood this waiver and agreed to its terms voluntarily
