  • HOW TO USE THIS FORM

  • A Region member (individual) or Region Club  may lodge a complaint by completing this Complaint Form as soon as reasonably possible following the alleged breach.

  • Unofficial Complaint (Report)

  • You may also use this form to report an alleged breach of ERBR policies. A "report" is an unofficial complaint, unofficial complaints are notifications and may or may not be actioned by the Board. If you want the Region to conduct an investigation or commence disciplinary action please make a formal complaint.

  • Who can use this this form

    • Any person who has seen something that the don't believe promotes the ideals of the Region or is a potential breach of Region policy.

    • This Complaint Form must be completed in full at the time of submission, where it is not, the Board is not obliged to process the Complaint. Acknowledgement of receipt of this form will be sent to the email provided in the complaint. An incomplete Complaint Form will not be actioned, the Complainant will be notified their complaint is incomplete or insufficient.

  • ERBR Championship Event where the Policy has been breached

  • Please indicate if this complaint is official or unofficial.

  • Official complaints will commence action regarding the alleged breach, unofficial complaints may not result in action however the report will remain on file and may be actioned if the Board believes action is required.*
