Enrollment Agreement Disclosure Statement
Do not sign this agreement before you have read it in its entirety or if it contains any blank spaces. This agreement is a legally binding document.
This agreement becomes binding when it is signed and dated by both the student and an authorized representative of Nursing Careers Institute.
Students should read the entire agreement carefully and ensure they understand all terms and conditions prior to signing. The student is entitled to receive an exact copy of the fully executed agreement and any disclosure statements signed.
This Enrollment Agreement, together with the school catalog and published institutional policies, constitutes the entire agreement between the student and Nursing Careers Institute.
Nursing Careers Institute does not guarantee employment or job placement upon completion of the program.
The institute reserves the right to reschedule a program start date if enrollment is insufficient or if circumstances arise that require schedule adjustments.
The institute reserves the right to terminate a student’s enrollment for reasons including, but not limited to:
- Unsatisfactory academic progress
- Nonpayment of tuition or fees
- Failure to comply with institutional policies or standards of conduct
- Participation in the clinical portion of the program is contingent upon the student meeting all program requirements, including but not limited to:
- Payment of all tuition and fees
- Submission of required health documentation
- Compliance with clinical facility requirements and policies
- Failure to meet clinical eligibility requirements may result in the student being unable to participate in the clinical portion of the program, which may delay program completion.
Nursing Careers Institute does not guarantee the transferability of any credits or training hours to another college, university, or institution. Any decision regarding the acceptance of credits or training hours is made solely by the receiving institution.
Tuition Obligation and Payment Responsibility
By signing this agreement, the student acknowledges responsibility for payment of all tuition and fees associated with the program. The student understands that tuition covers the cost of instruction and training only and does not guarantee successful completion of the program or passing of the state competency examination.
Tuition obligations remain subject to the school's published refund policy, and students are responsible for reviewing and understanding those terms prior to enrollment.
