Nursing Careers Institute and the student named below enter into this Enrollment Agreement under which the student agrees to pay the tuition and applicable fees as outlined in this agreement. By signing this agreement, the student acknowledges receipt of the school catalog and agrees to comply with the policies, rules, and regulations of Nursing Careers Institute.

Nursing Careers Institute agrees to provide instruction in the program curriculum as outlined in the catalog and in accordance with applicable regulations governing proprietary schools under the North Carolina State Board of Community Colleges.

The Nurse Aide training program is conducted in accordance with training standards established by the North Carolina Department of Health and Human Services Division of Health Service Regulation.

Refund Policy
Refunds will be calculated based on the student’s official withdrawal date, which is defined as the last date of attendance recorded by the school.

Students wishing to withdraw must submit written notice to the Program Director by completing the school’s withdrawal form. If a student stops attending without providing written notice, the withdrawal date will be determined by the student’s last documented date of attendance.

Additional Refund Conditions

• If the school cancels a program, students will receive a 100% refund of tuition and fees paid.
• Students dismissed due to misconduct, violation of school policies, or unsatisfactory academic progress will be considered withdrawn as of their last date of attendance, and refunds will be calculated according to the schedule above.
• Refunds will be issued within 30 days of the official withdrawal date.
• Important Notice:Tuition covers the cost of instruction and training only. Payment of tuition does not guarantee successful completion of the program or passing of the state competency examination.

I hereby acknowledge receipt of the school catalog, dated information about policies and procedures, programs offered, and equipment/supplies provided. I have carefully read and can make a copy of this enrollment agreement. 

I understand that the college may terminate my enrollment if I fail to comply with attendance, academic, and financial requirements or if I fail to abide by established standards of conduct, as outlined in the school catalog. While enrolled in the institute, I understand that I must maintain satisfactory academic progress as described in the school catalog and that my financial obligation to the institute must be paid in full before approval is granted to take the state exam. 

I understand that the institute does not guarantee job placement to graduates upon program completion or upon graduation. However assistance may be provided.

I understand that complaints which cannot be resolved by direct negotiation with the institute in accordance to its written grievance policy may be filed with the North Carolina Community College System. All student complaints must be submitted in writing. 

I have read and understand this agreement and acknowledge receipt of a copy. It is further understood and agreed that this agreement supersedes all prior or contemporaneous verbal or written agreements and may not be modified without the written agreement of the student and the Administrative Authority. I also understand that if I default upon this agreement, I will be responsible for payment of any fees or attorney fees incurred by Nursing Careers Institute. My signature below signifies that I have read and understand all aspects of this agreement and do recognize my legal responsibilities regarding this contract.

Enrollment Agreement Disclosure Statement

Do not sign this agreement before you have read it in its entirety or if it contains any blank spaces. This agreement is a legally binding document.

This agreement becomes binding when it is signed and dated by both the student and an authorized representative of Nursing Careers Institute.

Students should read the entire agreement carefully and ensure they understand all terms and conditions prior to signing. The student is entitled to receive an exact copy of the fully executed agreement and any disclosure statements signed.

This Enrollment Agreement, together with the school catalog and published institutional policies, constitutes the entire agreement between the student and Nursing Careers Institute.

Nursing Careers Institute does not guarantee employment or job placement upon completion of the program.

The institute reserves the right to reschedule a program start date if enrollment is insufficient or if circumstances arise that require schedule adjustments.

The institute reserves the right to terminate a student’s enrollment for reasons including, but not limited to:

• Unsatisfactory academic progress
• Nonpayment of tuition or fees
• Failure to comply with institutional policies or standards of conduct
• Participation in the clinical portion of the program is contingent upon the student meeting all program requirements, including but not limited to:
• Payment of all tuition and fees
• Submission of required health documentation
• Compliance with clinical facility requirements and policies
• Failure to meet clinical eligibility requirements may result in the student being unable to participate in the clinical portion of the program, which may delay program completion.

Nursing Careers Institute does not guarantee the transferability of any credits or training hours to another college, university, or institution. Any decision regarding the acceptance of credits or training hours is made solely by the receiving institution.

Tuition Obligation and Payment Responsibility

By signing this agreement, the student acknowledges responsibility for payment of all tuition and fees associated with the program. The student understands that tuition covers the cost of instruction and training only and does not guarantee successful completion of the program or passing of the state competency examination.

Tuition obligations remain subject to the school's published refund policy, and students are responsible for reviewing and understanding those terms prior to enrollment.
