  • Please fill in as much information as possible. Incomplete information may hinder our ability to investigate the incident. Any information you have to corroborate your narrative should also be attached to this document (i.e., addresses, photographs, screenshots, etc.)

    You may choose to report information anonymously. You can choose to be a confidential source. In this case, your personal information will be shared, but an investigator will contact you to gather more information about the incident. The police will also be notified of any incident that involves potential criminal behavior.    

    In the State of New Jersey, a person is guilty of hazing, a disorderly person’s offense, if they knowingly or recklessly organize, promote, facilitate, or engage in any conduct in connection with the initiation of applicants to or members of a student or athletic team organization, which places or may place another person in danger of bodily injury. A person is guilty of aggravated hazing, a crime of the fourth degree, if it results in serious bodily injury to another person.   

  • Background Information

  • The disclosing person does not need to be the harmed person and/or a Complainant. Saint Elizabeth University will review all disclosures to determine whether the conduct falls within the scope of this policy or other related policies.

    An individual may pursue a Saint Elizabeth response procedure independently of any off-campus processes, such as reporting to law enforcement or pursuing other non-campus-based civil reporting options. Saint Elizabeth University reserves the right to initiate this process even if law enforcement has declined to do so.

  • Definitions of these terms:

    Urgent & Important – This situation involves immediate safety risks or ongoing harm. It requires prompt follow-up within 24 hours.
    (Examples: threats, physical danger, severe emotional distress, potential criminal behavior.)

    Important but Not Urgent – This concern matters and should be addressed, but it does not present an immediate safety risk.
    (Examples: concerning group behavior, potential policy violations, need for follow-up or guidance.)

  • Involved Parties

    List all involved parties and/or potential witnesses.  An individual(s) listed as “Respondent” is the party that may have violated the policy. One listed as “Complainant” is the individual(s) to whom the misconduct was directed. One listed as “Witness” is the observer of the incident. One listed as “Reporting Party” is the person who informed you of the incident.
  • Questions

    Please provide a detailed description of the incident or concern using specific, concise, and objective language.  Please provide as much detail as possible about the behavior that prompted the report.  Details about any threats or harm to self or others, dates and times, or changes in behavior are helpful.  If you are concerned for the immediate health and safety of this person or others, please call 911.  You may also call SEU Security at (973-290-4090).  Security may also contact the police.
