Please sign below to indicate to request and authorise payroll deduction: I hereby request and authorise the employer to deduct from my wages/ pay the amount set by the Australian Workers’ Union of Employees, Queensland / The Australian Workers’ Union (AWU Queensland) in accordance with its rules as my annual membership, in regular instalments and remit it forthwith to the Union. This authority shall be deemed to remain in full force and effect until written revocation is given by me to my employer. The Union is authorised by me to direct my employer to; (a) vary the sum specified from time to time; (b) divulge any information the Union deems necessary to comply with the Union(s) rules and relevant State/Commonwealth Acts; (c) provide to the union updated information when my circumstances change; and (d) provide to the Union details of my financial institution (bank/credit union branch and account number) to enable completion of a Direct Debit Authority, if requested by me in writing.

I agree to become a member of The Australian Workers’ Union of Employees, Queensland registered under the Industrial Relations Act 2016 (Qld) and The Australian Workers’ Union registered under the Fair Work Act 2009 (Cth) (AWU Queensland), and abide by the rules of the aforesaid union and to any alterations/amendments made in conformity with the abovementioned Acts or in accordance with the Rules. PRE-EXISTING ISSUES - I understand that the AWU can only represent my interest from the date that membership is accepted. The AWU reserves the right not to represent a new application with a pre-existing issue. RESIGNATION FROM AWU - I understand that notifying my Delegates or ceasing to pay contributions does not terminate my membership of the AWU. A member can resign by notice in writing to the Secretary/Branch Secretary. The notice must be signed and then provided by mail, fax, email, or in person. Resignation generally takes effect from the date specified in the notice or two weeks after the notice is received - whichever is the later.
