Please complete the form below to request a dyslexia evaluation with Dyslexia School of Houston. For additional questions or concerns please contact evaluations@dyslexiahouston.org.

The total cost of the evaluation is $3,000. A non-refundable deposit of $1,500 is required to begin the evaluation process. The remaining $1,500 balance is due upon delivery and review of the final report.
