    Uploading your full Certificate of Insurance (COI) documentation is required to submit this form and complete the onboarding process.

    Please make sure your COI is complete and compliant with CPT’s requirements before submitting. Providing this information upfront helps prevent delays and allows us to review your application more efficiently.

    To be considered fully compliant and proceed with the onboarding process, you must upload the following:

    • General Liability
    • Auto Liability
    • Workers Comp

    Even if you are exempt from carrying Workers' Compensation, CPT requires coverage.     Each row represents a specific category of work (e.g., CCTV, Access Control, Fiber, Electrical). The columns represent different rate types:

    Standard Rate: Applies during normal business hours, Monday through Friday, 7:00 AM to 6:00 PM
    Non-Standard Rate: Applies outside of normal hours, Monday through Friday, 6:00 PM to 7:00 AM
    Emergency Rate: Applies during normal business hours (M–F, 7:00 AM to 6:00 PM) for same-day, short-notice dispatches
