ACVO/ABVO Committee & Board Member Document Submittal Form

Please be sure to pre-read/sign the required documents on the ACVO and ABVO Committee On-Boarding/Renewal pages. These forms will either need to be attached or acknowledged in the below form.

If signing the NDA, or Exam NDA, for the first time, a signed "wet" document is required. For subsequent years on the same committee, signed "wet" documents are not needed annually, but annual agreement and re-submission of the digital form below is REQUIRED for all Committee members. If changing to a new committee or board, all signatory documents need to be re-submitted and attached.
