This form is for parents or guardians who wish to request that their child/children not be filmed or photographed during Casey Basketball competitions, in line with Basketball Victoria's Photography Policy.

The duration of this exemption, if granted will apply until the child/children turns 18 or unless otherwise stated.

Section 4: Acknowledgement and Consent
By submitting this request, I acknowledge that:
- This exemption applies only to Casey Basketball competitions and does not extend to tournaments, representative programs, or other BV-sanctioned events unless separately requested.
- Every effort will be made to respect this exemption within Casey Basketball’s control, but incidental capture (e.g., in background footage or group photos) cannot be fully guaranteed.
- I understand this request does not apply to external team, association, or club social media content unless arranged with those parties directly.
