    This waiver must be completed before any services are provided. Signing is voluntary; however, refusal to sign will result in immediate appointment cancellation without refund.
  • 1. Booking & Payment

    • A valid card must be on file at all times to secure an appointment.

    • All appointments must be booked online—“walk-ins” are not accepted.

    • Payments must be successfully processed before services are rendered. If a payment does not go through, the client will receive an invoice and be banned from booking until the outstanding balance is settled.

    • All payments are final. Chargebacks or disputes against legitimate transactions will result in a permanent ban from booking. Any disputed charge that is ruled in favor of Chosen Massage and Wellness will result in additional legal and administrative fees.

     
    1B. Deposits & Outstanding Balances

    • If a partial deposit is paid and not the full service amount, the remaining balance will be charged to the card on file at the start of the appointment.

    • If payment fails, the appointment will be canceled immediately, and all previous payments forfeited.

    • Disputing or avoiding payment will result in permanent ban and additional legal/administrative fees.

    • Partial payment does not constitute full payment. Full service price is legally owed by booking.


    1C. Courtesy Appointment Holds
    As a professional courtesy, Chosen Massage and Wellness may temporarily hold an appointment slot upon client request. This is NOT a confirmed booking and does NOT guarantee the spot.


    • All hold requests must be sent via text to (615) 913-0742 for documentation purposes.

    • Held appointments for future dates must be fully booked and paid by 5:00 PM on the same business day as the request, or the hold will be released without notice.

    • For same-day appointments, the hold will only be honored until 1 hour before the appointment time, unless otherwise stated.

    • Repeatedly requesting holds without booking may result in temporary or permanent booking restrictions.


    1D. Right to Refuse Service (Including Groupon Bookings)

    Chosen Massage and Wellness reserves the right to refuse service to any client, at any time, for any reason deemed necessary by the business owner. This includes, but is not limited to, repeated miscommunication, inappropriate behavior, violation of booking policies, harassment, excessive cancellations or rescheduling, or failure to adhere to posted terms and conditions.

     
    This policy applies to ALL clients, regardless of how the service was booked or purchased—including Groupon and third-party vouchers. In the event that service is refused:

    • All scheduled appointments will be canceled.

    • All booking fees and deposits are non-refundable, including those made via Groupon or third-party platforms.

    • Future booking attempts will be automatically declined and blocked.

     
    By booking an appointment, clients acknowledge and agree to abide by all policies outlined by Chosen Massage and Wellness. Failure to do so may result in permanent denial of services.


    2. Cancellations & No-Shows

    • Cancellations/Rescheduling within 24 hours of the appointment and no-shows will forfeit all monies paid.

     
    For Example:

    If your appointment is at 12:00 PM on Wednesday, the latest you can cancel without penalty is 11:59 AM on Tuesday.

    • If you cancel at 12:01 PM on Tuesday, that is within 24 hours, and you forfeit your payment.

     • Cancellation Fee: All cancellations or reschedules made within 24 hours of the appointment, as well as no-shows, will be charged a $50 fee to the card on file. This fee applies to all clients, regardless of membership (members: $30 cancellation/late reschedule) or how the service was booked (including Groupon).


    • Same-day reschedules count as cancellations, meaning you would need to pay a new deposit to rebook.

    • No exceptions will be made for emergencies, illness, or unforeseen circumstances unless explicitly granted by Chosen Massage and Wellness in writing.

    • Late cancellation/rescheduling fees and/or no-show fees cannot be applied to future appointments. Clients must rebook and submit a new deposit.

    • Repeated no-shows or last-minute cancellations may result in a permanent ban from booking.

     
    3. Late Arrivals

    • A 5-minute grace period is allowed for completing necessary paperwork. This grace period does not extend your session time.

    • If you arrive past the 5-minute grace period, the lost time will be deducted from your session.

    • If you arrive late and still need to complete paperwork, the time spent doing so will be deducted from your appointment time.

    • If you arrive excessively late (15 minutes or more), without communication your session will be canceled, and all payments forfeited. No refunds will be issued, and forfeited payments cannot be used for future sessions. A new deposit is required to rebook.

     
    4. Early Bird and After-Hours Appointments

    • Early Bird appointments must be booked directly through (615) 913-0742. A $30 mandatory early bird fee applies and will not go toward your appointment price.

    • Early Bird is any appointment booked from 6:30 AM to 8:45 AM, Tuesday–Saturday.

    • Same-morning requests for Early Bird appointments are not accepted. Text by 8 PM the night before to confirm availability.

    • After-Hours Appointments must be booked directly through (615) 913-0742. A $50 mandatory after-hours fee applies and will not go toward your appointment price.

    • After-Hours is any appointment booked after 5 PM, Tuesday–Saturday.

    • Same-day requests must be booked by 4 PM. Latest appointment is at 7 PM.

    • Full payment including applicable fees is required to secure your spot.

    • Professional conduct rules apply at all times. Any inappropriate behavior will result in immediate cancellation, forfeiture of all payments, and permanent ban.

     
    5. Adolescent Appointments (Under 18)

    • A parent or legal guardian must be present for the entire session.

    • A signature is required before the service begins.

     
    6. Prenatal Massages

    • Expecting mothers must be at least 14 weeks along to receive a prenatal massage.

    • A doctor’s note may be required for high-risk pregnancies.

     
    7. Professional Boundaries & Conduct

    • Sexual harassment, advances, or innuendos are strictly prohibited.

    • Any inappropriate behavior will result in immediate termination of the session, full charge of service, and a permanent ban from booking.

    • This is a professional environment. Any violation of this policy will not be tolerated.

     
    8. Guest Policy

    • Only the client receiving the service is allowed in the treatment room.

    • The only exception is for adolescent clients, who must have a parent or guardian present.

     
    9. Liability Disclaimer for Personal Belongings

    • Chosen Massage and Wellness and Ashley Bacon are not responsible for lost, stolen, damaged, or misplaced personal items.

    • Clients are responsible for securing their belongings before, during, and after sessions.

    • Clients release Chosen Massage and Wellness and Ashley Bacon from any liability for loss or damage of personal property.

    • Not responsible for complications related to body piercings, modifications, or tattoos during/after session.


    10. Liability & Health Disclaimer

    • Not responsible for any sickness, injuries, allergic reactions, or health-related issues before, during, or after session.

    • Not medical professionals—do not diagnose, treat, or prescribe medical conditions.

    • Clients waive the right to pursue legal action for any injury, reaction, or health-related issue that may arise.

     
    10B. STRICT HEALTH DISCLOSURE POLICY

    For the health and safety of both the therapist and clients, you are REQUIRED to disclose ANY and ALL health conditions PRIOR to your appointment. This includes but is not limited to:

    Herpes (oral or genital), shingles, cold sores, rashes, lice, or any contagious skin or viral conditions. If you have herpes and are experiencing an outbreak (genital or oral), or if you currently have lice or any contagious condition, YOU MUST reschedule your appointment for a later date. This is non-negotiable. If you arrive and it is discovered that you are having an active outbreak or failed to disclose a condition, your appointment will be TERMINATED IMMEDIATELY and you will be charged IN FULL.


    Cancellation fees and policies still apply.

    This is a clean, safe, and professional space. There is ZERO TOLERANCE for dishonesty or putting others at risk. Respect the space and the safety of your therapist and fellow clients.

     
    11. Legal Protection & Jurisdiction

    • Any disputes arising from services provided by Chosen Massage and Wellness will be governed by Tennessee state law.

    • Legal action must be filed in Davidson County, Tennessee.

     
    12. Faith-Based Practice

    • This is a faith-based business. Only gospel music will be played, including worship, gospel R&B/hip-hop, and gospel instrumentals.

     
    13. Groupon Appointments 

    Clients booking using a Groupon Voucher are held to the exact same standards and policies as regular clients. By booking a Groupon massage with Chosen Massage and Wellness, you agree to the following terms without exception:

     
    • Groupon massages MUST be booked through the “Groupon” tab only. [Unless your Groupon says “with membership”] 

    If you book a Groupon massage under any other tab, your appointment will be treated as a regular full-price service. Your Groupon will not be honored for that session. You may either:

    • Request a refund directly through Groupon Customer Service or

    • Save the voucher for a properly booked appointment in the future.

     ** Requesting a refund via Groupon Customer Service doesn’t mean it’ll get granted** 

    • A valid card is required to book a Groupon appointment.

    A $10 non-refundable retainers fee will be charged to your card to confirm the booking. A valid card must remain on file at all times. Appointments made without a card on file will not be honored.

     
    • Groupon clients are held to the same cancellation, rescheduling, and no-show policy as all other clients.

    This means:

    • Cancellations or reschedules made within 24 hours of your appointment will result in a $50 fee charged to the card on file.

    • No-shows will also be charged a $50 fee to the card on file.

    • If the card is declined or the balance is not paid, the client will receive an invoice and be banned from future bookings until the outstanding balance is settled.

     
    • The Groupon voucher must be entered correctly and will be marked as redeemed upon booking even in the event of a late cancellation, no-show, or any violation of the policies listed. Failure to do so will result in an invalid appointment and may lead to cancellation. The voucher will not be reinstated or refunded by Chosen Massage and Wellness. 

     
    13B. Misbookings and Incorrect Groupon Purchases:

    Misbookings will not be honored. For example, purchasing a prenatal massage Groupon (intended only for pregnant/postnatal clients) to receive a cheaper deep tissue massage will result in cancellation. Using the wrong Groupon to receive a discount violates booking policies. No refunds will be issued. Clients are responsible for ensuring the Groupon purchased matches the service booked. Please review all descriptions and policies before scheduling.

     
    14. Membership Policy

    Memberships at Chosen Massage and Wellness are billed monthly and include benefits as outlined at the time of sign-up.


    • You may cancel your membership anytime after the first billing cycle. Cancellations must be submitted via text to (615) 913-0742 before the next billing date to avoid being charged for the upcoming month.

    • If your first billing payment fails (due to a declined or locked card, or any other reason), you will be invoiced immediately for the full membership amount.

    • Clients with unpaid membership invoices will be suspended from booking until the balance is paid in full. (See 16B for more information about invoices) 

    • All memberships are non-transferable and non-refundable.

    • Rollovers are only valid with an active membership. If a membership payment is missed or canceled, any unused rollovers are forfeited. Rollovers cannot be transferred, redeemed for cash, or saved if the membership ends. Consistently missed appointments may also result in membership cancellation.

    •$30 late cancellation/late reschedule fee will be charged to the card on file, if you cancel within the 24 hour cancellation window (see section 2)

     
    15. Spa Party / Group Bookings 

    • A non-refundable retainer of 50% of the total cost is required to secure your spa party date.

    • You may also choose to pay in full upfront at the time of booking.

    • The remaining balance must be paid no later than 24 hours before the scheduled event.

    • Example: If your spa party is on Saturday at 2 PM, your full balance must be paid by Friday at 1:59 PM.

    • If full payment is not received by then, you do not have an appointment, and your spot will be forfeited.

    • The quote is based on the number of guests you reserve for.

    • Example: If you book for 10 people and only 8 show up, you will still be charged for 10. No exceptions.

    • No-shows or missed services will not be rescheduled or credited. They must book separately at regular service rates.

    • All standard terms, cancellation, rescheduling, and late policies still apply. — including early bird / after-hours appointments. 

    • All payments are strictly non-refundable, regardless of circumstances


    16. Agreement Enforcement

    • By booking an appointment, you acknowledge that you understand that deposits/retainer fees are non-refundable and that you have read, understood, and agreed to these terms and conditions, regardless of whether a signature has been provided.

     
    16B. Invoices & Collections Policy

    All balances must be paid within 5 calendar days of the service date.

    • A $35 late fee will be added every 7 days the balance remains unpaid.

    • If a balance remains unpaid after 60 days it will be sent to a third-party collections agency.

    • A $30 administrative fee will be added to cover processing and recovery costs.

    • Interest and additional fees may be applied by the collection agency once the debt is transferred.

    • Clients with outstanding balances cannot book future services until the account is paid in full.

    • Payment arrangements must be made before the 5-day grace period ends.


    By booking with Chosen Massage and Wellness, you acknowledge and accept these terms in full, whether or not a signature is obtained.
