This form is to be completed for families wanting to request a change to current class enrolments.

This applies for students who;

need to change class enrolments due to new circumstances in their family routine

have recently had a birthday and are now required to train in a new age group
eg: new age is now 8 years or 14 years (or senor instructor has recommended this change)

have had a belt change and are now at a new developent level
eg: new belt is now orange, blue, brown/white or brown

This form will guide you through the enrolment process based on the age and belt level of the student.
