  • At Trinity's Day Spa, our primary goal is to provide entertainment through our various services. We prioritize safety and client satisfaction, and we'd like to share some important information with you:

    Service Approach: We do not utilize powered equipment, cuticle tools, or harsh chemicals in our services, ensuring a gentle and enjoyable experience.
    Allergic Reactions: While we strive to use products safe for children, it's important to note that allergic reactions can occur. 
    Liability: Trinity's Day Spa cannot be held responsible for any allergic reactions, skin damage, or any other adverse reactions, including death, resulting from our services or related activities, including costumes or food and beverage consumption.
    Dietary Restrictions: Please inform our staff of any dietary restrictions to ensure your safety and enjoyment during your time with us.
    Safety: Our staff is highly focused on providing a royal and secure environment for your event and guests. Your safety and satisfaction are our top priorities.

    By engaging in our services, you acknowledge and accept these terms and policies. We look forward to providing you with a delightful experience at Trinity's Day Spa.
     

    I, the undersigned, parent/guardian, assume all risk and hazards incidental or participating in the activity and hereby waive, release, absolve, indemnify, and agree to protect, defend, and hold harmless Trinity's Day Spa, their owners, staff, and participant(s) for any claim arising out of any injury to myself/child to the fullest extent allowed by law.

    I, the undersigned, parent/guardian of the participant(s), am fully aware of the potential dangers and risks inherent in this activity, including physical injury, death, or other consequences that may arise or result directly or indirectly from participation in all spa and spa party activities. If any provision of this clause is held unlawful, void, or for any reason unenforceable by a court of competent jurisdiction, then any such provision shall be deemed severable from this clause and shall not affect the validity and enforceability of the remaining provisions.

    Your signature below acknowledges that you are the responsible party and that you have read and agreed to this waiver/release form harmless clause.

  • Trinity's Day Spa Policy Contract

  • Payment: Please book your party 2 weeks in advance with a $100.00 non-refundable deposit that will be applied toward your balance. The remaining balance must be paid in full 7 business days prior to the party date.

    Cancellations: Once full payment is received, your payment becomes non-refundable and the date and time of the scheduled event become unchangeable, except for extenuating circumstances such as a power outage.

    If you are more than 15 minutes late for your appointment/event, your appointment/event may be canceled or (with approval of management) rescheduled. This may occur due to the time constraints of finishing your appointment/event during the allotted time, so as not to inconvenience another appointment/event taking place after yours. Any rescheduling may be possible per approval with management.

    Food: There is No Food allowed in the building, except for cake or cupcakes provided by the parent.

    Clean-up: Trinity's Day Spa is not responsible for the setup or cleanup of the Party Room. Parents/Guardians will be responsible for setup and cleanup, but will be provided with an 8-foot table along with chairs in the party room.

    Weapons: All weapons - including concealed weapons - must remain outside of our facility.

    Photography/Videos: We may use photos or videos taken during your event for promotional purposes or to post on our website/social media sites. If there are any children who are NOT allowed to be photographed or recorded, please list their first name below.

     

    Occupancy: Adult supervision: All appointments require at least one adult to accompany the children throughout the service. For all birthday parties, a maximum of 2 adults can be present at a time.
    Restricted access: Only those receiving services are allowed in the designated service area.

    Liability: Trinity's Day Spa and its owners and staff are not all licensed salon professionals. When booking your party, as a client, you agree and understand that all services are for entertainment purposes only. We do not promote brand-name or licensed characters as part of our party packages. A further detailed description of our liability policies can be found under the waivers and release form.

    Please be aware that the signer of this contract is responsible for informing all parents/guardians of this agreement and its requirements. Your signature below acknowledges that you are the responsible party and that you have read and agreed to Trinity's Day Spa policies.
