Please ensure you complete this form with accurate and complete information, as this is critical for proper commission reconciliation and timely commission payments. A separate form is required for each distinct travel component booked. For example: If you booked a cruise and a hotel separately (two different bookings), please submit two forms—one for the cruise and one for the hotel. If you booked a cruise with a flight through the cruise line and all components are listed under one confirmation number, then you only need to submit one form.
