Ashley Glover and the undersigned podcast guest for participation in "The Perspective Shift" podcast. 1. Acknowledgment of Podcast Purpose & Nature By signing this Agreement, the Guest acknowledges and agrees to the following: Informational & Entertainment Purpose: The Podcast is for informational and entertainment purposes only. Its aim is to provide new perspectives, facilitate self-reflection, and explore various life challenges. Not Therapy or Professional Advice: The Host is not a licensed therapist, psychologist, medical professional, legal professional, or financial advisor. The conversations on this Podcast are not a substitute for professional psychological, medical, legal, or financial advice, diagnosis, or treatment. No Professional-Client Relationship: No professional-client, therapist-patient, or doctor-patient relationship is established or implied by the Guest's participation in the Podcast. Guest Responsibility: The Guest understands that they are solely responsible for their own well-being and any decisions or actions they take as a result of their participation or listening to the Podcast. If the Guest is experiencing a crisis or requires professional help, they are strongly encouraged to seek advice from a qualified professional. Focus on Perspective Shift: The Podcast focuses on helping individuals gain new viewpoints, reframe issues, and foster self-awareness. It does not provide direct solutions, clinical diagnosis, or treatment. 2. Consent to Record and Use Content The Guest hereby grants the Host and its assigns, licensees, and successors the irrevocable, worldwide, royalty-free, perpetual right and license to: Record: Record the Guest's voice, statements, likeness, and any other audio or visual contributions ("Contributions") during the podcast recording session(s). Use & Distribute: Use, reproduce, publish, distribute, broadcast, transmit, and display the Contributions, in whole or in part, in any and all media now known or hereafter devised, including but not limited to, podcast episodes, promotional materials, social media, websites, and derivative works. Edit & Modify: Edit, modify, adapt, or combine the Contributions with other material, provided that the Host shall not materially alter the meaning or intent of the Guest's statements. No Compensation: The Guest understands and agrees that no monetary compensation will be provided for their participation or the use of their Contributions. 3. Protection of Guest's Identity & Privacy The Host commits to the following measures to protect the Guest's identity: Anonymity: The Guest's actual name will not be used during the Podcast episode. A pseudonym will be used, agreed upon prior to recording. Details Alteration: The Host may, with the Guest's prior consent, alter minor identifying details within the Guest's story (e.g., specific locations, names of minor characters, non-critical professions) to further protect anonymity, without changing the essence of the story or the issue discussed. Confidentiality of Personal Information: Any personal contact information (e.g., email, phone number) provided for scheduling purposes will be kept confidential by the Host and will not be shared publicly without explicit written consent from the Guest. Limitations of Anonymity: The Guest acknowledges that while efforts will be made to protect their identity, complete anonymity cannot be absolutely guaranteed in a public forum. 4. Host Protections & Disclaimers No Liability: The Guest agrees to release and hold harmless the Host, its owners, employees, contractors, and affiliates from any and all claims, liabilities, damages, and expenses arising from or in connection with the Guest's participation in the Podcast or the use of their Contributions. Views Expressed: The Guest understands that the views and opinions expressed by the Guest on the Podcast are solely their own and do not necessarily reflect the views or opinions of the Host or "The Perspective Shift" podcast. Right to Refuse/Terminate: The Host reserves the right to refuse to record, or to discontinue recording, any session, or to decline to publish any episode, at its sole discretion, especially if a topic is deemed outside the scope of the Podcast or ethically inappropriate. 5. Guest Representation The Guest represents and warrants that: They are at least 18 years of age and have the full legal right and capacity to enter into this Agreement. Their Contributions will be original to them and will not infringe upon the rights of any third party. Their Contributions will not be defamatory, obscene, or violate any applicable laws. By signing below, the Guest acknowledges that they have read, understood, and agree to all the terms and conditions of this Consent and Release Form.
