This form is to be completed by the School Sports Coordinator or convenor for any event or issue arising from or as a result of a SACSA sporting competition. If a first Aid Officer is present, they may choose to fill out their own Sporting Incident Report Form and provide copies to the SACSA Executive Sports Coordinator or Convenor
