Informed Consent and Acknowledgement
(for Woodwork Sessions with Smart Hands Studio)

I give permission for my child to participate in all woodwork activities organised by Smart Hands Studio during their booked sessions.

I understand that while every effort is made to ensure a safe environment, woodwork activities carry some inherent risk, including the possibility of splinters, grazes, bruises, or small cuts. By giving this consent, I acknowledge and accept these risks.

In agreeing to my child’s participation, I assume full responsibility for any such minor incidents and agree to release and hold harmless Smart Hands Studio, its staff, and session leaders from any claims arising out of participation in these activities.

This consent covers participation in all activities as part of the registered programme and is given voluntarily for the duration of my child’s involvement.

Medical Authorisation and Emergency First Aid Consent
(for Woodwork Sessions)

As the parent and/or legal guardian of the named child, I give permission for basic first aid to be administered during woodwork sessions if needed.

This includes, but is not limited to:

Cleaning of minor wounds
Applying plasters
Use of cold compresses or ice packs for minor bumps or injuries
I understand that this consent allows Smart Hands Studio and its representatives, including facilitators and session staff, to provide appropriate first aid in the event of a minor injury.

This authorisation applies for the duration of the child’s registered participation in woodwork sessions and is given to support the safety and wellbeing of my child in my absence.

Confirmation

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
