PARENT’S NIGHT OUT PARTICIPATION RELEASE FORM
By my signature below, for myself and/or as the parent and/or legal guardian of the child(ren) listed below, I hereby:
(i) agree to assume all risk of injuries, whether minor, serious, or catastrophic in nature, that may result from my child(ren)’s participation in the activities at Purple Dragon Atlanta’s Parent’s Night Out, whether allegedly resulting from my negligence or the alleged negligence of Purple Dragon Atlanta, Purple Dragon International, or any of their staff, agents, representatives, or instructors; and,
(ii) agree, acting for myself, my heirs, personal representatives, and assigns, to release Purple Dragon®, Purple Dragon International, and Purple Dragon Atlanta, its staff, agents, representatives, and instructors from all liability, including claims and suits at law or in equity for any injury, fatal or otherwise, that may result from my child’s participation in Parent’s Night Out activities, whether allegedly resulting from my negligence or the alleged negligence of Purple Dragon Atlanta or any of their staff, agents, representatives, or instructors; and,
(iii) represent and warrant by my signature (each parent and/or guardian present must sign below) that I am the legal parent or guardian of the child(ren) listed below and that I have read and fully understand each of the above conditions and agree that Purple Dragon® and Purple Dragon Atlanta may act in reliance upon this waiver in allowing my child(ren) to participate.
Parent’s Night Out Guidelines
- Parents are required to provide emergency contact information and remain reachable during the event.
- Purple Dragon Atlanta reserves the right to dismiss any child whose behavior endangers themselves or others.
- Please ensure your child uses the restroom prior to drop-off.
- Children must be signed in and out by a parent or authorized guardian. If someone else will pick up your child, please inform us in advance.
- Prompt pick-up is required at 9:00 PM. Additional fees may apply for late pick-ups.
- Purple Dragon Atlanta is not responsible for lost or stolen personal belongings.
- Snacks, pizza, and drinks will be provided. Please notify us of any allergies or dietary restrictions prior to the event.
- Participation in activities such as games, martial arts drills, and movie time is voluntary but encouraged.
- Purple Dragon Atlanta maintains a respectful environment. Students and families are expected to uphold our values of kindness, respect, and integrity.
