By submitting this form, you acknowledge and agree to the following terms as a condition of participating in Pride n' Play: Adult Field Day, hosted by Harmonizing Masculinity Collective (HMC):

Assumption of Risk

You understand that participation in outdoor recreational activities involves inherent risks, including but not limited to physical injury, illness (including COVID-19 or other communicable diseases), dehydration, exposure to weather conditions, slips, falls, and other potential hazards.You voluntarily assume all risks associated with your participation in the Event.

Release and Waiver of Liability

You hereby release, discharge, and hold harmless Harmonizing Masculinity Collective (HMC), Transinclusive Group, Sunserve, Girls On Gyals, its staff, community partners, volunteers, sponsors, vendors, agents, and all affiliated parties from any and all liability, claims, demands, causes of action, or expenses (including attorneys' fees) arising from your participation in the Event, including but not limited to claims for personal injury, illness, property damage, or wrongful death, whether caused by negligence or otherwise.

Medical Authorization

You authorize HMC representatives to provide or arrange for first aid and/or emergency medical care in the event of illness or injury. You understand you are solely responsible for any associated medical costs.

Photography & Media Release

You consent to the use of your image, likeness, and voice in photographs, video, and other media taken during the Event for promotional, educational, and fundraising purposes, without expectation of compensation.

Rules & Conduct

You agree to comply with all Event rules and instructions provided by HMC staff and volunteers. You understand that failure to comply may result in removal from the Event without refund.
