Health & Vaccinations
: I certify that my dog(s) is in good health and has not been ill with a communicable condition in the last 30 days. I have provided up-to-date proof of vaccinations, including Rabies, Bordetella, and DHPP.
Medical Care Authorization:
Should my dog(s) become ill or injured during boarding, I authorize Pack Nine LLC to seek treatment. I agree to be responsible for any and all veterinary costs incurred.
Behavior & Safety:
I confirm that my dog does not have a history of aggressive behavior toward people or other animals. I understand that if my dog displays aggressive or destructive behavior, it may be isolated or refused service.
Personal Belongings:
I understand that any personal items (toys, bedding, etc) left with my dog are left at my own risk. Pack Nine LLCis not responsible for loss or damage to these items.
Assumption of Risk:
I understand that there are inherent risks when dogs interact with other dogs and that injuries or illnesses may occur. I release Pack Nine LLC, it's owners, employees, and agents from any liability arising from my dog's participation in boarding or related activities.
Abandonment:
I understand that if I do not pick up my dog within 5 days of the agreed-upon pickup date and cannot be contacted, my dog may be considered abandoned and handled in accordance with local animal control laws.
Photo/Video Release:
I grant Pack Nine LLC Permission to take and use photographs or videos of my dog for promotional or social media purposes. I understand that no personal information will be shared.
