PLEASE READ ALL TERMS AND CONDITIONS
The effective date of this agreement is the date of signature and submission to Mays Plastic Surgery and The Spa at MPS.
Membership Term
1. This Agreement will operate on an ongoing month-to-month basis, with a minimum commitment of three consecutive months (90 days). After three months, the patient is able to cancel this membership with a 7-day cancellation notice.
2. If a patient cancels their membership within 90 days of the effective date, the patient is required to pay back any discount(s) received during that time period.
3. Members are eligible to freeze their membership only after maintaining an active membership for a minimum of three consecutive months. A membership may be frozen once every six months, for a duration not exceeding three months per freeze period.
4. By signing this agreement, the patient agrees to authorize Mays Plastic Surgery and The Spa at MPS to charge the card on the patient’s file on the above-mentioned day of each month and amount.
5. All monthly memberships are paid in advance monthly, on the calendar date the membership has started, for the following month.
6. This Agreement is personal to the patient and may not be assigned or transferred.
Termination or Suspension of Membership
1. Please be advised that you have the right to cancel this Agreement at any time, after your first three consecutive months have been paid, by contacting our office at least 7 days before the next monthly charge is due.
2. Mays Plastic Surgery and The Spa at MPS reserves the right at any time to cancel or suspend the membership of any patient in the event of the following:
a. The member commits a serious breach of this Agreement and/or Mays Plastic Surgery and Med Spa Rules and Regulations.
b. Where any monies are due to Mays Plastic Surgery and The Spa at MPS by the patient remain unpaid for 30 days after its due date for payment.
c. The member knowingly provided false details when applying for membership and the false declaration would have reasonably affected Mays Plastic Surgery and The Spa at MPS’s decision to grant the membership.
d. If Mays Plastic Surgery and The Spa at MPS terminates for any reason, they reserve the right to retain any monies received to cover any reasonable costs they have incurred as a result.
Terms and Conditions
1. Mays Plastic Surgery and The Spa at MPS Gift Cards may not be redeemed to pay any portion of the membership fee. The membership fee cannot be combined with any other promotion and/or discount.
2. Mays Plastic Surgery and The Spa at MPS reserves the right to vary, add, or eliminate any of the services within this membership at any time.
3. Patients cannot transfer or gift services of the membership to another patient.
4. Unused credit balances within this monthly membership cannot be refunded.
5. Unused credit balances at the end of the month will be rolled over each month. There is no limit to how many months can be rolled over.
6. Surgery consultations and surgery fees are not eligible to receive discounts.
7. Credit balances may be used towards tips for select services.
8. Credit balances may be used to purchase other promotions at The Spa at MPS.
