TRINITY EPISCOPAL CATHEDRAL RELEASE

INSTRUCTIONS: If you are under 21 years of age. both you and your parent (or guardian) are to sign and date the Release. If you are 21 years of age or older, only your signature and date are required. The Release and signatures need not be notarized.

In furtherance of its ministry and to afford opportunity to the undersigned and other members of the Trinity Episcopal Cathedral community ("Trinity"), Trinity sponsors and makes available mission, pilgrimage and other off- campus travel opportunities, as well as events and activities on the Trinity campus. In exchange for and in consideration of this sponsorship, the undersigned participant agrees to release Trinity, its officers, vestry members, instructors, mission/pilgrimage trip leaders, volunteers, employees, agents, successors, and assigns from any responsibility for, and to indemnify each thereof against and hold them harmless from, all obligations, liabilities, claims, demands, costs, and expenses, including reasonable attorney's fees, arising out of or in any way connected with any of the following:

(1) Any and all claims of whatever nature for any injury, loss, damage, accident, delay, irregularity, or expense arising a) from the use or operation of any vehicle or services, b) from strikes, acts of terrorism, anarchy, kidnapping or hostage taking, political/social movements, war, weather, sickness, quarantine, government restrictions or regulations, c)  from any act or omission of any steamship, airline, railroad, bus, transportation, sight-seeing, hotel, or any other service or transporting company, firm, individual, or agency, or d) from any other cause whatsoever in connection with the respective event or activity;

(2) Any intentional or unintentional injury to the participant, caused in whole or in part by the participant or any other person or persons;

(3) Any intentional or unintentional damage or injury to property, whether personal, real, or mixed, owned by or in the custody or possession of the participant or others, caused in whole or in part by the participant or any other person or persons; and 

(4) Any contractual or other financial obligations incurred by the participant during any trip or other activity, including without limitations, obligations, or liabilities incurred by the participant in any foreign country or location in which the trip or event takes place.

The undersigned participant hereby consents and agrees not to sue or bring suit or legal or equitable action against Trinity or any of its officers, vestry members, instructors, mission/pilgrimage trip leaders, volunteers, employees, agents, successors, and assigns for or on any matter covered by the foregoing releases and indemnities.

This form when completed will be utilized for all Trinity Cathedral events for the 2025-2026 Program Year. You need only to submit this form once during this time. Please contact your program area administrator for questions.

IN WITNESS WHEREOF, the undersigned has hereunto affixed his/her/their signature.

RIGHT FOR TRINITY TO USE PHOTOS, VIDEOS, AND/OR RECORDINGS: I hereby authorize and release Trinity to record my/my child's likeness, via still photo, video, or audio recordings, for use as promotional material for Trinity and/or its programs. I understand that these recordings may be edited at the discretion of Trinity, and that they may be published, including without limitation in promotional videos, brochures, Trinity websites, and social media platforms. I hereby waive all rights to compensation of any nature for the use of these photos, videos and/or recordings.
