  • When to Use This Incident Report Form

    This form should be completed by Match Officials whenever an incident occurs at a match. It is used to report a range of issues that may impact the game. Please use the form to report any of the following:

    • Incident Reports – Including verbal abuse, physical altercations, or player/official injuries.
    • Ground Conditions – Such as weather-affected pitches, unsafe or unsuitable fields, or field marking problems.
    • Match Abandoned – Where a match is not completed due to reasons like team no-shows, severe weather, or spectator interference.

    Prior to submitting an Incident Report, the author of the Incident Report must confirm the identity of the Participant with the Participant or with the Team Official of the Participant’s Team.
