I am the parent or legal guardian of the minor child named above and hereby give my full consent for their transportation with Meridian Limo LLC – Viae Group (including its brands and affiliates, such as Meridian Limo and Green Ride Limos).
I acknowledge that transportation involves certain risks, and I voluntarily assume all responsibility for any injury, illness, or incident that may occur during or related to the ride. I hereby waive, release, and hold harmless Viae Group, its officers, agents, contractors, and affiliates from any and all liability, claims, or causes of action arising from the transportation of my child, including while entering, exiting, or riding in the vehicle.
I certify that I am legally authorized to give this consent and that I have reviewed the nature of the transportation service. I agree to notify the provider of any relevant medical, behavioral, or logistical considerations related to my child prior to the trip.
By signing below, I confirm that I have read and understood this waiver and release and agree to its terms voluntarily and without duress.
