  • PPBCC SPAY AND NEUTER WAIVER FORM AGREEMENT

    Please read and understand the following information.
    • I am allowing PHILIPPINE PET BIRTH CONTROL CENTER FOUNDATION INC. to neuter my dog/cat.
    • My companion has undergone 6-8 hours of fasting prior the surgery preparation.
    • Vomiting is a sign that your companion animal did not undergo fasting and this may result to choking and suffocation.
    • For companion animals 2 years old and below, CBC is required to ensure that my cat/dog is fit for surgery.
    • For companion animals 3 years old and above, CBC with blood chemistry is also required to ensure that my cat/dog is fit for surgery.
    • If my pet falls under the following categories;

      -Small Breeds (Regardless of the weight)
      -Large Breeds (Regardless of the weight)
      -Mixed Breeds (Regardless of the weight)
      -Pets 3 years old and above
      -Senior Pets
      -Pets with past health complications

      I am aware that they are required (Against Medical Advice Waiver is not applicable) to have the following services done before and during the surgery;

      -Complete Blood Count
      -Blood Chemistry (Critical Care)
      -Gas Anesthesia

    • If I wish to have my companion animal proceed with the surgery without bloodwork done I will sign and understand the Against Medical Advice waiver. (Applicable to pet 3 years old below & doesn't have previous health condition; For pets 3 years old above mandatory services is needed) 
    • I understand that my cat/dog will undergo anesthesia and other medications needed to perform the neuter surgery.
    • I understand that the anesthesia will cause reactions and my companion needs to rest for a few days to fully recover.
    • I understand that my animal companion may wake up later than the given estimated time of pick up.
    • I understand that if my animal companion does not fully recover from the anesthesia two (2) hours past the estimated pick up time, that they will stay at the clinic overnight to recuperate and will be released the following day. 
    • My companion is in good health, has not been sick for the past 3 weeks and he/she is physically fit for the surgery.
    • I understand that withholding any medical information may lead to complications before, during, and after my companion animal's surgery.
    • I will administer the prescribed medication after the surgery.
    • I understand that failing to give the prescribed medication may lead to infection and complications.
    • My cat will have an ear tattoo/notch as a marking that they are already neutered.
    • I am aware that ear markings are required for my pet's safety.
    • Neutering (kapon) is a surgical procedure wherein the uterus and ovaries are removed from the female animal while the testicles are removed from the male animal to prevent them from unwanted reproduction.
    • Neutered animals will not be capable of reproduction. The male cat/dog may still have the desire to mate due to their memory of the behavior; there is still a possibility that after seven (7) days, the male may still get female cats/dogs pregnant due to their reserved sperm.
    • If there's unusual bleeding, wound infection, lethargy, loss of appetite / drinking, or vomiting, I will immediately proceed to Philippine Pet Birth Control Center Foundation.
    • I will not hold Philippine Pet Birth Control Center Foundation Inc. liable if I don't raise any complications that arose immediately.
    • I agree to continue the surgery if my companion animal has HYDROMETRA, PYOMETYRA or is PREGNANT, and which will be known only when the surgery has started. I am aware that this comes with additional charges.
    • I agree to continue the surgery if my companion animal has undescended testicles, which includes additional charges.
    • I will put my companion animal in a secure carrier to ensure their safety and comfort. Failing to use the proper carrier will lead to the cancellation of my appointment.

    CARE AND GUIDELINES FOR COMPANION ANIMALS BEFORE AND AFTER SURGERY

    • Your companion will undergo anesthesia. After the surgery, give them plenty of time to rest to recover.
    • Cats/dogs should not be given food and water after the operation. Allow 3-4 hours to pass before giving moderate amount of food and water.
    • Your companion may display inactivity for two (2) days after the operation.
    • Cats/dogs should not be bathed for fourteen (14) days after the operation.
    • Do not let your companion jump, run, and play with other animals for five (5) days.
    • Avoid letting them lick, bite, or play with their wound. Use an Elizabethan collar (E-collar) to be safe.
    • Be sure to give the prescribed medication to avoid any infection.
    • Kindly place the E-collar on your companion after the surgery.
    • Read aftercare and guidelines given upon release of your companion.
