By signing below, you’re saying you understand and agree that:
• My pet is up-to-date on all required vaccines (rabies, distemper/parvo, bordetella).
• My pet is healthy and flea/tick free.
• I’ve shared any behavior concerns or special needs honestly.
• I understand there may be a fee for late pick-up, extra care, or emergency situations.
• I give permission for Home Away From Home staff to get veterinary help in an emergency — and I’ll cover any costs.
• I understand that, while every effort will be made to keep my pet safe and happy, accidents or illness can happen. I release Home Away From Home Doggy Daycare and Boarding, LLC from liability in such cases.
