This form is to be completed by meet directors no later than 2 weeks prior of your meet. If you submit within 2 weeks of your event, and rush shipping is needed, you will be charged the rush shipping fees for your packet.
Starting 2/25/2025, we will be using new Chain of Custody drug test forms. With these forms, there is no need for a separate Athlete Notification Form to be used. It is built into the new CoC form.
Starting 5/9/2025, we will no longer be sending out the drug test instructions with your meet packets. If you need a copy of the instructions for your competition they can be found and printed at the link below.
Starting 5/30/2025, we are no longer accepting checks for the drug test payments. I has to be done electronically through the results form in order for your results to be submitted.
