This form is to be completed by meet directors no later than 2 weeks prior of your meet.  If you submit within 2 weeks of your event, and rush shipping is needed, you will be charged the rush shipping fees for your packet.

Starting 3/14/2026, we will be using new (Orange) Chain of Custody drug test forms. With these forms, you will need to use the Athlete Notification Form. 

We will no longer be sending out the drug test instructions with your meet packets. If you need a copy of the instructions for your competition, they can be found and printed at the link below.

Please, only fill out this form for meets that you already have sanctions for. These supplies are costly, and we don't want to just needlessly hand out overstock. Thank you for your dedication to USA Powerlifting. 

If you have more than 4 meets that you would like supplies for, please fill out another form.

 

We will send out a sufficient amount of supplies for each meet all packaged together. Depending on how many supplies are needed you may receive one box with the cups and one with the forms and bags.

We document all supplies that we send out and to whom, so if you end up expanding your meet to more than what it was origanally sanctioned for after these supplies have been sent out, you will be responsible for the shipping cost. You will need to email bjfaas@usapowerlifting.com for an invoice to pay first. You can also reach out to the National office.
