I, the undersigned participant (or parent/guardian if under 18), hereby acknowledge that participation in the softball tryouts involves physical activity and carries inherent risks of injury. I voluntarily assume all risks associated with participation, including but not limited to, falls, contact with other participants, equipment failure, and weather-related hazards. I release and hold harmless the coaching staff, organizers, facility owners, and all affiliated personnel from any and all claims, liabilities, or damages arising from or related to my (or my child’s) participation in the tryouts.

I also grant permission for the tryout organizers to seek emergency medical treatment on my behalf (or on behalf of my child) in the event of an injury or illness and affirm that I (or my child) am physically fit to participate.
