I have provided to the supplier company the following evidence of current proof of competence for myself and/or at least one of our employees :

1: CRRU UK approved training certification which is less than 5 years old.
or
2: CRRU UK approved training certification which is 5 years old or more and proof of current membership of a CRRU UK approved CPD scheme.

I confirm that all orders placed by any employee of our company, for delivery to our main office address or any alternate address will only be used by a currently Certificated User acting on the behalf of our company.

I recognise that it is my responsibility to immediately notify the supplier company of any changes to named Certificated Users, e.g. if they leave the company.
